Admissions Procedure Checklist

STEP I

• Read enclosed information carefully.
• Call to schedule introductory interview and tour of school
• An meeting will be scheduled for interested parents/guardians to meet with Pastor.

STEP II

• The parent/guardian will be notified regarding status of acceptance.

STEP III

• The following must be returned for each prospective student:
                          ___      Completed Registration Form
                          ___      Record of immunization, health form, and medical administration form (if applicable)
                          ___      Reference letter from Pastor (non-member)
                          ___      All forms must be completed and signed
                          ___      Any records from other daycare providers or pre-schools (if applicable)
                          ___      Nonrefundable $75 Application Fee
• Registration for payment arrangements will be completed.
